Who is responsible for your information

Information we use and why
We receive information when you join the practice and, where applicable, when another clinician refers you. This includes your contact details and information about your health and dental care. We use it to arrange and provide care, keep a record of your treatment, and communicate with you about that care. Without the information needed for an assessment, we may be unable to provide appropriate treatment.
For care provided under our agreement with you, we rely on the UK GDPR lawful basis of performance of a contract. We process health information under the condition for providing health or social care or treatment. We do not send marketing emails or text messages to patients.
Where the law requires us to retain or disclose information, we rely on compliance with that legal obligation. We do not use consent as the basis for keeping the clinical information needed for your care.
Who we share it with
Members of your care team use your information to provide treatment. Service providers working under contract with the practice may process it on our behalf. If we refer you to another practitioner or hospital, we will discuss the referral with you and obtain your agreement before sharing the relevant information. We may also have to disclose information where the law requires it.
Where records are stored
Our clinical records are held in DentalPlus and stored in the UK. This statement applies to clinical records; information sent through this website is covered by the website privacy policy.
How long we keep records
We retain clinical records in line with applicable UK legal and professional requirements. How long a record needs to be kept depends on the type of record, whether it relates to an adult or a child, the date of the patient’s last care, the need for continuity of care, and any relevant complaint or legal obligation. Your rights
You can ask for access to your personal information and ask us to correct information that is inaccurate. Depending on the circumstances, you may also be able to ask us to restrict processing, receive a copy in a portable format or erase information. Clinical records may need to be kept even if you ask us to delete them. You can object to processing by contacting us; we will explain how your rights apply to the information concerned.
