Twin Block braces: how they work and what to expect

A Twin Block is a removable orthodontic appliance used for some growing children whose upper front teeth sit well ahead of the lower teeth. It has separate upper and lower parts with angled blocks that guide the lower jaw forwards when the mouth closes.

It is one option for a particular bite problem, not a treatment for every type of crooked tooth. An orthodontic assessment is needed to decide whether it is suitable and when to start.

How does a Twin Block help?

The appliance changes how the teeth and jaws meet while it is worn. Treatment can reduce the gap between the upper and lower front teeth, known as the overjet. Tooth movement contributes to the result; the appliance does not guarantee a particular amount of jaw growth or a permanent change in facial appearance.

Timing depends on your child’s growth and dental development as well as the bite. A fixed age range such as eight to fourteen is not enough to decide suitability. Starting as early as possible does not always shorten the whole course of treatment.

Fixed braces are often needed afterwards to align individual teeth more precisely. Ask the orthodontist to explain both stages and the likely need for retainers at the end.

Wearing the appliance

Follow the wearing schedule given by your orthodontist. This usually involves substantial daily wear, including at night, but instructions about meals and the exact number of hours can differ. Do not use another child’s instructions as a substitute for your own plan.

Speech and swallowing may feel different at first, and there can be some initial soreness. Tell the team if there are persistent ulcers, significant pain or problems fitting the appliance. They can check the fit and help your child manage wearing it. Do not force a damaged appliance into place or try to adjust its wires yourself.

Cleaning and everyday care

Brush teeth thoroughly with fluoride toothpaste and clean the appliance as shown by the orthodontic team. Avoid hot water, which can distort it. When it is out of the mouth, keep it in its protective box rather than wrapped in a tissue where it can be thrown away.

Ask when to remove it for swimming or contact sports and what mouth protection to use. If it has an adjustment screw, only turn it when and how the team has specifically instructed you.

Attend the planned reviews so the orthodontist can check the fit and progress. If wearing it is difficult, raise that early. Treatment time varies with the starting bite, growth, response and wearing pattern; a short advertised timescale is not a guarantee.

What does Twin Block treatment cost?

Our listed Twin Block appliance fee is £600 for both arches. It is not a separate charge for each arch and should not be read as the cost of every stage of orthodontic treatment.
